PURPOSE OF THIS CONSENT
Montebello Real Estate Brokers Associates Inc. ("the Agency") is committed to protecting the privacy and confidentiality of your personal information in accordance with applicable federal and provincial laws, including the Civil Code of Québec and Québec's Act Respecting the Protection of Personal Information in the Private Sector (Law 25).
This consent authorizes the Agency to collect, use, disclose, retain, and destroy your personal information as necessary to provide real estate brokerage services and fulfill its legal and regulatory obligations.
COLLECTION OF PERSONAL INFORMATION
I understand and authorize the Agency to collect personal information that may include, but is not limited to:
• Full name;
• Residential and mailing address;
• Telephone numbers;
• Email addresses;
• Date of birth;
• Government-issued identification;
• Financial and banking information;
• Employment information;
• Credit information and credit reports;
• Information relating to a real estate transaction;
• Information required to comply with anti-money laundering laws and regulatory requirements;
• Any other information reasonably necessary for the provision of real estate brokerage services.
USE OF PERSONAL INFORMATION
I authorize the Agency to use my personal information for the following purposes:
• Providing real estate brokerage services;
• Evaluating my eligibility to buy, sell, lease, or rent real estate;
• Preparing offers, leases, contracts, and related documentation;
• Conducting identity verification and due diligence;
• Obtaining credit reports and references where required;
• Communicating with me regarding real estate transactions;
• Complying with legal, regulatory, insurance, accounting, and professional obligations;
• Protecting against fraud, identity theft, money laundering, and other unlawful activities;
• Maintaining records required by the Organisme d'autoréglementation du courtage immobilier du Québec (OACIQ).
DISCLOSURE OF PERSONAL INFORMATION
I understand that my personal information may be disclosed, where necessary, to:
• Other real estate brokers and agencies involved in a transaction;
• Buyers, sellers, landlords, tenants, and their representatives;
• Financial institutions and mortgage lenders;
• Notaries, lawyers, inspectors, appraisers, and other professional advisors;
• Credit reporting agencies and background verification providers;
• Government agencies and regulatory authorities, including the OACIQ;
• Service providers retained by the Agency who require access to the information to perform their services.
The Agency will only disclose personal information to the extent necessary for the purposes described above or as required by law.
RETENTION AND PROTECTION OF INFORMATION
The Agency will retain personal information only for the period required by applicable laws, regulations, and professional obligations.
The Agency will implement reasonable administrative, technical, and physical safeguards to protect personal information against unauthorized access, use, disclosure, alteration, loss, or theft.
CLIENT RIGHTS
I understand that I have the right to:
• Request access to my personal information;
• Request correction of inaccurate information;
• Withdraw my consent, subject to legal or contractual restrictions;
• Obtain information regarding the Agency's privacy practices;
• Submit a complaint concerning the handling of my personal information.
Requests concerning personal information may be directed to the Agency's Privacy Officer.
CONSENT
I acknowledge that I have read and understood this Consent Form.
I voluntarily consent to the collection, use, disclosure, retention, and destruction of my personal information by Montebello Real Estate Brokers Associates Inc. for the purposes described herein.
I understand that refusing to provide certain information may prevent the Agency from providing some or all requested services.
